Prevent 'featured breach' results on subsequent email scans.

Changes conditional in scan-form.hbs to scan form to make sure that feautred breach results are only returned when a user scans their email from a featured breach "/?breach=" page and not on scans initiated from results pages that contain a featuredBreach listing.
