Forms and controllers updated: coll.num, spl, and sequences

Fix forms and controllers to pass params through form_with model:
via hidden fields. (Previously they were added to the form_tag url.) Add params to whitelisted in controller.

Sequences and collection_numbers controllers: sort out lines mis-allocated between split new/create edit/update methods. Restore private methods build_sequence, build_collection_number, and save_edits.

Also small tweaks to image_thumbnail, image_vote_links,
Thumbnail helper: max height:width proportion is now 2:1 (from 3:1)

Pivotal index: switch @stories.count to @stories.size
Herbarium_records form - switch .count to .size
