Add Sepolia deployment (#65)

Adds deployment information on Sepolia.
The deployment was done by checking out `v1.0.0`, installing, adding the
Balancer vault Sepolia address, and running the deploy script.

Additionally, I added some details on how to verify the vault relayer
code to the readme, as well as the necessary new package.

The network RPC for Sepolia was found
[here](https://chainlist.org/chain/11155111).

### Test Plan

See that the contracts have been deployed to the expected deterministic
addresses and that the contract code has been verified on Etherscan.
