chore: Clean up `Cancel()` handling in TeeReader.

Make sure that calling `Cancel()` on the reader results in
`errc::operation_canceled` for the callers. Also make sure that
subsequent calls after this have no effect.

To do this, make sure to clear `pending_read` after calling handlers,
to signal whether or not an operation is in progress.
